Program Coordinator

 SKYMA Living Solutions is seeking a detail-oriented and compassionate Program Coordinator to support the planning, coordination, and daily operations of our community-based care programs. This role is essential in ensuring smooth service delivery and positive client experiences across our meal delivery, home care, and transportation services. 

• Coordinate program activities, schedules, and resources
   
• Serve as a liaison between clients, caregivers, and administrative staff
   
• Assist with client intake, documentation, and care plan tracking
   
• Monitor service delivery and follow up to ensure quality and satisfaction
   
• Maintain accurate records and generate reports as needed
   
• Support outreach, enrollment, and client engagement efforts
   
• Collaborate with other departments to ensure seamless operations
   
• Identify areas for program improvement and assist with implementation
